The applicant must present one document from each of the following categories:-

Evidence of UK business identity 

For Limited Company  
The original or certified copy of the company's Memorandum and Articles of Association OR an original or certified copy of the Certificate of Incorporation.  

OR

For Public Limited Company  
Certificate to Commence Business 

Evidence of personal identity - Certificate holder - primary document -  one of the following

• Current signed passport
• Debit or credit card with photo
• National Identity Card or Armed Forces Identity Card with photo
• Full UK driving license with photo

Evidence of personal identity - Certificate holder  - secondary document- one of the following 

• Full UK driving license with or without photo - if not used for as primary document 

• Bank statement/Building Society/Mortgage Statement (issued within last 3 months) 

• Council tax assessment (issued within last 3 months) 

• Utility statements for gas, water, electric, phone or mobile phone (issued within last 3 months) 

• Vehicle Registration Document 

• Motor Insurance/House Insurance Certificate
